St. Isidore Faith Formation - Instructions for Registering

Please follow these steps to register:

  • Print out one copy of the Registration Form & ER Consent Form for each child.
  • Completely fill out all forms by hand and sign where necessary.
  • You can pay by either check (made out to St. Isidore’s) or by printing out and completing the Credit Card Authorization form.
  • Please consider volunteering, there are many options listed on the Volunteer form.
  • Mail or drop off completed registration packet to the Faith Formation Office.
    • A completed packet contains:
      • Registration Form (one per child)
      • ER Consent Form – 2 pages (one per child)
      • Payment
      • Volunteer Form (optional)
